NonStop's Client based in the North West of England is seeking a number of Social Care Assessors to join their Hospital Team.

As a Social Care Assessor, your role would be to work in collaboration with health partners to support hospital discharge.
To be considered for this post you should be have good organisation and time management skills. Relevant experience in similar roles is desirable. Also a plus is previous experience of working in a health or social care environment or similar setting including working with people with adult care needs in their own home.
