Social Worker Adult Assessment Team South Gloucestershire
Social Worker required for work within the Adult Assessment Team in the South Gloucestershire area. Rates are £30ph – 32ph depending ion experience. 6 month initial contract. Exciting opportunity in a friendly and supportive team. Based at the Badminton Road Offices. Hybrid working between office and home.
An excellent opportunity has arisen for a Social Worker within Adult Social Care, where you will be responsible for carrying out SW needs assessments and providing advice and guidance to our service users. You will liaise and work closely with contacts from different sectors, health and the voluntary sector, working as part of a multi-disciplinary team, ensuring that communication is effective, and that knowledge and best practice are shared.
Responsibilities include:
* reviewing and monitoring care plans and the quality of service provision and respond to changes of need. This includes developing and evaluating reablement programs as part of care packages and ensuring that programs to support independence are implemented.
* managing your own caseloads or specific projects to ensure specialist inputs are obtained and effective outcomes achieved.
* raising awareness of a range of care issues and their implications, carry out risk assessments and complete safe methods of work practice guides, in order to ensure that needs are assessed, and appropriate action is taken.
You will be Social Work England registered, ideally a driver with access to own car and able to reside and work in the UK
