Job Description:
Mobilising packages of care and managing a challenging caseload while always operating in line with both the Care4ocus Healthcare and the NMCs codes of conduct.
Assessing new packages of care and working with the mobilisation team to build a safe package of care for clients in the community.
Working with the mobilisation team to rapidly respond to clients’ needs and new packages of care (within 24 hours) Supporting End of Life Packages of Care in the community.
Liaise with CCGs, Hospitals and Local Authorities and be able to communicate effectively with a variety of personalities within each.
Working with the Clinical Director, Head of Clinical Mobilisation and National Clinical Lead to ensure that the service being offered to stakeholders is consistent across all Care4ocus Healthcare services.
To ensure clients who use Care4ocus Healthcare services do so safely and expediently, providing a high level of patient care
To be approachable, clear, and coherent, self-motivated, persistent and have a passion for providing a quality nurse lead service
Interviewing of staff and bespoke recruiting/screening for packages of care
Building holistic, person-centred care plans and risk assessments, adhering to GDPR regulations when handling confidential information and documents.
Specific Requirements:
You must be registered as an RSCN / RN or Dual registered
Must have ITU/HDU/PICU/NICU/A&E experience
Must be able to work flexibly responding to Urgent Mobilisation of Packages
Support in Training of Staff Health Care Assistants and working shifts until competent & signing off competencies (Mentorship/Training qualification desirable)
You must have Tracheostomy and Ventilator experience
Minimum 5 years post registration experience, 2 years of these in Complex Care environment.
Attend the Harrow office at least once per week
Car Driver with Clear Driving Licence, prepared to travel long distances with possible overnight stays (funded)
Having the ability to work calmly under pressure
Being able to manage your own diary
Registered enhanced DBS on the update service.
