They work in partnership with public and private sector healthcare companies to offer life-changing treatment to patients in their own homes. We are actively supporting them in the recruitment of Nurses to work delivering care to patients in their own home as part of the early discharge team.
THE RESPONSIBILITIES
The overall responsibility of this position is to deliver effective and high-quality nursing care to patients in a home environment to facilitate early supported transfer from hospital or to prevent unnecessary admission. Your day to day will involve visiting patients in their own homes and safely delivering the required care to them based on their individualised care plans. You will work as part of a multidisciplinary team of nurses, physiotherapists, OT's and social care workers. Your role will involve administering a range of therapeutic interventions, updating their care plans, answering any queries or concerns they may have, assisting patients in their activities of daily living and support multi-disciplinary goals of optimising patients' level of independence. You will have an innate ability to quickly identify and report any risks and incidents. You will be fully aware of the principles of safeguarding as they apply to all vulnerable adults and children in relation to the specific role.
REQUIREMENTS
Registered General Nurse with NMC pin number
Experience in an acute care setting post registration including some time taking independent clinical decisions and supervising the work of others
Experience in the care of IV devices and administration of IV therapies
Be a caring and compassionate nurse with a strong attention to detail
Experience of undertaking holistic clinical assessments
