Job Description
If you are based in Durham and have experience as a healthcare assistant, coordinator, or community services professional, in the NHS or elsewhere, Leaders In Care want to hear from you. We are looking for a Recovery Coordinator to work with service users within a Prison for males in Durham, Northeast, supporting their social integration and enabling them to lead meaningful and purposeful lives.
This is an incredibly important, challenging and satisfying role as Rehabilitation Coordinator, and it comes with a starting salary negotiable on experience, circa £19,000 – £23,250.
This is a full time, permanent position, working 37 hours per week – Monday to Friday – no night shifts or weekends required! Hours are circa 8am-4pm / 9am – 5pm.
You will be working within a busy / faced paced environment where no day is the same. You will be assessing males coming into custody, conducting 1-1's, formulating care plans and risk assessments.
As Rehabilitation Coordinator, you will be working as part of a multi-disciplinary team to support service users from the first contact and through their treatment/recovery journey.
Amongst your many duties, you will be providing assessment, recovery planning and onward referral, and helping to reduce drug and alcohol-related harm in terms of individuals and the wider community.
This is a very varied role, but in a typical week you would have been involved in some of the following matters:
Looking after your assigned groups of service users, including 1-to-1 work and therapeutic interventions
Providing screening, asset mapping, recovery planning and reviews
Capturing timely and accurate data in online resources and reports
Assessing new service users in terms of risk and special needs.
You will also be involved in triage assessment and onward referral to a range of treatment/community support agencies.
You will need to be creative, innovative, empathetic, and assertive to meet the needs of the service user, promoting access to treatment and engaging proactively with underrepresented communities. You will also need to carry out key harm reduction strategies such as running of the Needle Exchange and distribution of Naloxone, after completion of appropriate training or demonstration of relevant experience.
