Qualified Social Worker – Kirklees Council, Yorkshire – Adult Social Care – Up to £40ph!
Applicant needs to have at least 3 years SW experience working with people with Learning Disabilities in a Social Work Team.
Undertaking social work assessments under the Care Act and undertaking MCA and BI work.
As a member of a team, the jobholder is responsible for the holistic assessment of young person and their carers' needs, arranging and/or delivering services to meet these needs and reassessing needs as appropriate. The job holder will be supporting the young person to seamlessly transition from children services into adult services, ensuring that there is an holistic assessment of needs which has a clear emphasis on developing a young person's independence as they enter adulthood.
There is a particular focus on preparation for adulthood and supporting the young person to develop independence, make decisions for themselves and striving to support the young people to secure desired personal outcomes.
The post holder must have a good understanding of the Education, Health and Care plan process and be willing to attend and contribute towards the reviewing and monitoring of this plan.
You will be responsible for the day to day management of those cases for which you are the named worker. You will intervene with legal and procedural authority to protect vulnerable people at risk of abuse or neglect; to help individuals, families and groups to sustain and develop their capacities for independent living and in complex situations to try to prevent or resolve conflict which may place individuals at risk. You will be provided with professional supervision.
